Returning and New Prentice Student Registration

2010-2011 Registration Forms must be received by August 2, 2010.

 

Returning Prentice Students: Click on FILES BELOW to download the registration packet for the 2010-2011 school year. Print the documents, fill them out using a blue or black pen, and mail the COMPLETED packet to Prentice.

 

New Prentice Students: Click on FILES BELOW to download the registration packet for the 2010-2011 school year. New students must also complete and submit an “Authorization for Release of Cumulative Records” form, found under the supplemental forms section. Print the documents, fill them out using a blue or black pen, and mail the COMPLETED packet to Prentice.

 

BOTH Returning and New Prentice Students: Please review the supplemental forms section for any forms that may apply (i.e. Prescription Medication, Before/After School Clubs/Care, etc.) Any applicable form under the supplemental forms section should be completed and submitted along with the other 2010-2011 Registration Forms.

 

Attention: Please leave ALL “Teacher” fields blank as you complete the registration forms as classes have not been assigned!
